Admissions Procedure

Basic Education Departement

Republic Act (RA) 10157 also known as the “Kindergarten Education Act”, and its Implementing Rules and Regulations (IRR) state that enrolment in Kindergarten “shall be made mandatory and compulsory for entrance to Grade 1”, effective School Year 2012 – 2013.

At SFAC, the Kindergarten level specified in RA 10157 and its IRR refers to the Kindergarten level, and not the Pre- Kindergarten level, which is also offered at SFAC/SAS.

As per DepEd policy, only children who are 5 years old are eligible for enrolment in Kindergarten classes.

Age Requirements:

Nursery          

  • at least 3 years old on the opening of classes until the last day of the third month after the opening of classes of the intended year of enrolment.

Scenario: If the opening of classes is June 1, the child should be at least 3 years old within June 1 to September 30.

 

Pre-Kinder

  • at least 4 years old on or before the third month of the school year commencing from the opening of classes.

Kindergarten 

  • at least 5 years old on or before the third month of the school year commencing from the opening of classes

Grade 1          

  • must have passed the Kindergarten program at SFAC or its equivalent level from other DepEd recognized schools. Age must be 6 years old on or before the third month of the school year commencing from the opening of classes. Child must have a valid DepEd-issued Learner Reference Number (LRN).

While age is a major consideration for admission, the learner’s level of readiness remains more crucial. Therefore, aside from the age requirement, the pupil-applicant has to pass the respective social, emotional, physical and academic assessments prescribed by SFAC for each preschool level, before being admitted to the SFAC Preschool Department.

  1. Original Report Card (if applicable)
  2. Original Certificate of Good Moral Character (if applicable)
  3. PSA Certified Birth Certificate (Photocopy, bring original for verification)
  4. Two copies of recent identical 2×2 ID picture in white background
  5. Accomplished Application Form
  6. Parent’s Interview
  7. Entrance Examination
  8. Two (2) long brown envelopes

Steps:

  1. Go to the Admission Office to inquire on the schedule of entrance test and availability of slot.
  2. Pay the testing fee at the Cashier’s Office.
  3. Take the entrance test on the scheduled date.
  4. Interview with the Guidance Counselor/Guidance Advocate
  5. Interview with the School Principal or his/her duly authorized representative
  6. Issuance of Certificate of Acceptance signed by the Principal
College
  • Students, regardless of sex, age, nationality, religious belief, or political affiliation are welcome to Saint Francis of Assisi College. This is in compliance with the Philippine Constitution as well as the laws of the country and within the framework of the mission statement of Saint Francis of Assisi College.
  • A student’s application for admission will be duly processed when the applicant complies with all the academic requirements and qualifications set forth by the College, as prescribed by the Commission on Higher Education (CHED).
  • The College, however, reserves the right at any time to refuse or to admit students under certain conditions.
    • The College reserves the right to accept or refuse the application of students.
    • The College reserves the rights to drop or dismiss those who make false statements in the application for admission.
  • Senior High School graduates who were not able to enter College right after graduation for one semester or more must submit a certification from their principal stating that their Form 137 has never been released or forwarded to any other school.

Freshmen

  1. True copy of Form 138 (Official Grade 12 School Card)
  2. Photocopy of PSA authenticated Birth Certificate
  3. Four (4) copies of recent 1” x 1” colored ID pictures
  4. Certificate of Good Moral Character

Transferees

  1. Official Transcript of Records or Copy of Grades
  2. Photocopy of PSA authenticated Birth Certificate
  3. Honorable Dismissal or Letter of Recommendation (from the Registrar, Academic Chair or Chair of Student Affairs)
  4. Four (4) copies of recent 1” x 1” colored ID pictures

Degree Holders

  1. Original Copy of Transcript of Records (TOR)
Graduate School

Master of Arts in Education major in Educational Management

  1. graduate of a Bachelor’s Degree in Education (whether in Elementary or in Secondary Education) with a General Weighted Average (GWA) of 85% or General Point Average (GPA) of 2.25.
  2. at least two (2) years of teaching experiences;
  3. at least two (2) Recommendation Letters from Superiors;
  4. for graduates of non-education programs, eighteen (18) units of undergraduate professional education courses are required prior to admission; and
  5. for students from the public schools, a copy of the Study Permit is required for each of the semester/summer enrolment.

 

Master in Business Administration (Non-Thesis) Program

  1. graduate of any Business Program or its Allied Program with a General Weighted Average (GWA) of 85% or General Point Average (GPA) of 2.25;
  2. preferably with two (2) years industry work experience;
  3. at least two (2) Recommendation Letters from Superior and Character Reference; and
  4. for graduates of non-business programs, twelve (12) units of business preparatory courses are required before acceptance to the regular master’s program.
  1. Accomplish the following forms from the Graduate School Office: Application for Admission and Student Information Sheet (SIS).
  2. Submit the accomplished forms with the following attachments: 
    • Transcript of Records (TOR) from the school last attended. (TOR must be a copy for Saint Francis of Assisi College.)
    • Recommendation Letters
  3. Undergo an Interview with the Dean / Program Director.
  4. Upon approval for enrollment of the Dean / Program Director, secure Pre-Enrollment Form (PEF) from the Registrar’s Office.  Accomplish the PEF; refer to the List of Course Offerings to be enrolled.
  5. Submit the accomplished PEF for advising by the Program Director. Academic advising is based on the student’s Record of Academic Progress (RAP) and is a part of the Student’s Individual Folder (SIF) filed at the Registrar’s Office. 
  6. Submit the advised PEF to the Registrar’s Office for the issuance of the Certificate of Matriculation (CM) Form.
  7. Submit the CM for assessment and payment of fees to the Accounting Office for validation.
  8. After Payment of Fees, present validated CM to the Registrar’s Office for the issuance of valid blank course card per course enrolled.
  1. Secure Student Clearance from the Registrar’s Office and have it cleared by the indicated Offices.
  2. Submit accomplished Student Clearance to the Registrar’s Office for the issuance of the Pre-Enrollment Form (PEF).
  3. Accomplish the PEF; refer to the List of Course Offerings to be enrolled.
  4. Submit the accomplished PEF for advising of the Program Director. Academic advising is based on the student’s Record of Academic Progress (RAP) and is a part of the Student’s Individual Folder (SIF) filed at the Registrar’s Office. 
  5. Submit the advised PEF to the Registrar Office for the issuance of the Certificate of Matriculation (CM) Form.
  6. Submit the CM for assessment and payment of fees to the Accounting Office for validation.
  7. After Payment of Fees, present validated CM to the Registrar’s Office for the issuance of valid blank course card per course enrolled.

Enrollment Procedure

Basic Education Department

Steps:

1. Submit the following admission credentials to the Registrar’s Office.

    • Duly accomplished Application Form with one 2”x 2” colored learner’s ID picture in white background
    • Clear photocopy of birth certificate issued by the National Statistics Office (NSO) or Philippine Statistics Authority (PSA). Present the original copy of files for verification
    • Certificate of Good Moral Character issued by the principal or guidance counselor of the previous school
    • Entrance Exam Results / Certificate of Acceptance issued by the Admission Office
    • Latest original report card (Form 138) from previous school
    • Results of National Achievement Test for grade 7 and National Assessment Examination for grade 11, if applicable.
    • ESC ID / QVR Certificate For incoming grade 11 students from private schools with government voucher.

For foreign students:

  • Clear photocopy of passport. Present the original for verification.
  • Clear photocopy of Alien Certificate of Registration i-Card (ACR). Present the original for verification.
  • Notarized letter of request from the student- applicant for the processing and release of Special Study Permit addressed to the Bureau of Immigration, with a statement that all documents submitted were legally obtained from the corresponding learning institution.
  • Certificate of Clearance from the Bureau of Immigration.

2. Proceed to the Admissions Office

  • Fill out the Student Enrollment Form;
  • Submit the Student Enrollment Form for encoding; and
  • Obtain a printout of the Registration Form.

3. Proceed to the Accounting Office

  • Have the fees assessed based on the grade level and payment terms;
  • Pay the tuition and other fees; and
  • Have the Registration Form stamped “OFFICIALLY ENROLLED”

 

  1. Proceed to the Admissions Office
    • Fill out the Student Enrollment Form;
    • Submit the Student Enrollment Form for encoding; and
    • Obtain a printout of the Registration
  2. Proceed to the Accounting Office
    • Have the fees assessed based on the grade level and payment terms;
    • Pay the tuition and other fees; and
    • Have the Registration Form stamped “OFFICIALLY ENROLLED”.
